Yep...they take the penis, stretch it out as far as possible, then take a soda can and wrap the penis around it, so then the penis' skin is tight enough to properly tattoo.

I don't know about a back tattoo...but my two arm tattoo's didn't hurt much.

The only way that it could get infected is if the peircing isnt stablized or the area that the peircing is gets cut somehow and infection gets into the wound
