No I think it happens inside, the first issue cover I saw had these two face to face yelling at each other. I assume this is from the first issue, so I doubt it's the cover...way to graphic to be anyways

No, it is not impossible within the confines of comic book science. His bones are coated with adamantium. Not his ligaments and tendons, etc. The Hulk just dislocated severed his spine, disconnecting two of the vertebrae, and since the rest is just skin and organ, he tore it apart and tossed it.

Was the adamantium bonded in Ultimate Wolverine in different way then it was bonded in 616?

616 was different from Aoa in that AOA his bones could be severed, 616 as has been shown, they haven't been able to yet.

Isn't the current bonding Wolverine has by Apocalypse? So it might be somewhat different then Weapon X one.
