When a master does this, do they call out into the force and ask the force for a Darth name for their apprentice? So the force chooses it and not the master? or does the master just come up with any random name?

The Sith Master selects a name him/herself as befits the personality, behaviour or situation of the new Sith apprentice. Or even selects one that is ironic to the new Sith's situation or circumstance (for example: Vader means father in Dutch).

The name Darth is said to be a Sith title which simply means "shadow warrior". This has been borne by most Sith since (apparently) Darth Bane.

I think Maul is as in "to eviscerate" and "Bane" as in "something harmful or baleful."

Maul= to handle roughly
Bane= to kill especially with poison

Palp's sith name is Sidious, meaning Insidious, meaning decieteful and the like. Bane is harm or horror I think, Maul is self-explanitory. Like when you get mauled by a tiger.

Malak and Revan screwed up the continuity factor of the title "Darth."

But Revan and Malak were always Revan and Malak, they just added Darth in front of their name. Their names mean nothing since they have had those names all their lives.
