Shien is holding the lightsaber horizontally. Adi Gallia used this form.

Form X: Niman / Jar'Kai
Jar'Kai is a lightsaber form weilding two lightsabers. Asajj Ventress used this form.

Trŕkata
This lightsaber form is where the weilder of the lightsaber holds the lightsaber inactivated and strikes/ignites when a opening in the enemy's defense appears. Corran Horn used this form against the Yuuzhan Vong

Telekinetic lightsaber combat
Pretty much self-explainatory. Kreia displayed use of this form by levitating three lightsabers.

It was Han. Then George Lucas screwed it up by making Greedo shoot first. May seem like a minor thing to change, but it was an important scene to show how dirty and underhanded Han was before he met Luke.

According to starwars.com it's just called Hoth not Hoth VI, so I was right.

Actually all the planets were refered to as Hoth but since Hoth VI was the more famous and that was how they refered to the planet. Simular to Yavin IV. But if you said Yavin you would instantly think of the moon and not the gas giant.

Someone needs to tell the official Star Wars site that.

In real-time, it first appeared in A New Hope as the Millennium Falcon. In chronological order, it first appeared in Attack of the Clones.
