Cletus is more of your deep south serial killers who do it because "it's pretty."
Joker is insane AND dangerous - Carnage is dangerous predominantly because of his symbiote. Without the symbiote, he's just another run-of-the-mill serial killer, not better and no worse.
Joker, however, is not now, never has been and never will be any ordinary serial or mass murderer. He is a criminal mastermind, an insane genius, an inspired leader - and as mad as a hatter!
