Thanks to the extremis virus, Tony has like a supercomputer-like nervous system, giving him wayyyy faster than human reation time. Super-reflexes, if you will. Add that to the abilities of the Iron Man armor, and that he's a high level Class 100 lifter in the armor, and you get a shit-stomped Mr. Sinister.

In seriousness, the old Sinister, before he could be choked by Colossus and killed by Mystique apparently, would have offered a good fight.

He can regenerate from a puddle, had high level TK and TP, and energy blasts iirc. Apart from the two instances mentioned above I don't think I ever saw him actually being hurt.

Even back then Scott's optic blasts were the only thing that could cause lasting damage. It was retconned as a trick of Sinister's later on anyway IIRC.
