I never understood why people think of the Joker as a clown.
He wears the make-up of a clown, kind of. He isn't and never was an actual clown, though.

Well, he clearly holds the appearance of a clown. His most popular nickname is the "Clown Prince of Crime."
Also, some of his gadgets resemble that of what a clown would use. The acid-spitting flower on his coat and the hand buzzer are two examples.
Of course, these devices would only spit water and cause a small shock with a traditional clown whereas his are more dangerous.
