So why was dent mad at Gordon. What exactly did he do. He said it was his fault. I still dont get it.

Also, how does dent not realize its the Joker in the hospital until he takes the mask off.

Basically, Dent became a crazed man who couldn't be reasoned with by people with undamaged minds. I think that's why Joker's way of thinking made so much sense too him. The Joker just took a guy who had been going against him hardcore, created Two-face with the explosion, and then set the broken dude against his rival. Although i'm sure he was actually meaning too kill him, he saw a better opportunity in taking advantage of his condition.

1. Dent accused Gordon of having dirty cops working for him earlier in the film. It turned out Dent was right, and those dirty cops working for Gordon were involved in kidnapping Rachel, so Dent was mad at Gordon for not doing anything about his dirty cops that helped lead to Rachel's death. So Gordon was partially responsible for Rachel's death.

2. Dent had just had his world turned upside down. He had a ton on his mind, therefore would be just kind of spacing out and not paying close attention to everybody that walks into the room.

Gordon was the key link to stopping corruption in the force which directly led to Rachels death. The fact that only Harvey was punished for everyone elses failure in the battle against The Joker just made Harvey see red. Simple really. Harvey was effectively dumped all over, and he wasn't happy about certain other people getting off light and not shoulderring the responsibility - in this case Gordon, and Batman (when he showed up).

The snake tongue of The Joker got to him after he went ballistic at Gordon. The anger had made him weak and easy pickings for The Joker not necessarily to mould him, but to simply refine and focus his anger into something far more destructive. Gordon failled to make Harvey see reason, and that was possibly the pivotal moment in the film, was that was where Gordon lost the fight and The Joker won - and only Batman at the end understood this and agreed to preserve Harveys reputation. So really, Harvey just had all of his aggression directed toward Gordon because they both, along with Batman, ultimately failled in a high stakes game against Joker, but Harvey lost everything, while Gordon would go back to his cushy promotion behind a desk, and Batman would go back to being Batman - as if Harvey was going to let that slide. His role as he saw it was more like an angel of death, duplicitous soul given the responsibilities of dishing out punishment to those he felt deserved it as much as him.
