After Revan regained his memories did he go back to using the name Revan or did he stick with whatever name he had been going by prior to that?

No, there hasn't. And it's likely that if there will be, they'll stick to Revan. His alter ego is whatever millions of people wanted it to be, so how could they choose one and stick with it? That and everyone's now accustomed to him as "Revan". To avoid confusion, I wouldn't give him anything else.
