I think it would be a mistake to try to confront Russia about its nuclear posturing. I think we need to take the Teddy Roosevelt mantra to heart in this situation, carrying a big stick but speaking softly. The US needs to remind Russia that the Cold War is over, and that however much we might disagree over issues like Syria and Ukraine, it's in neither country's interest to play a game of nuclear chicken.
We need frank discussions with Russia over its nuclear activities. Russia has shown that it doesn't really respond to threats that well (nor does it respond well to capitulation), so we need a balanced approach that demonstrates we're not intimidated by Russia while also avoiding provoking Russia to acting out further out of fear.

Ugh, I just wish Putin would get out of office, he's a lunatic.
Edit: But hypothetically if Russia continued to advance its nuclear capability more, the U.S. would have to also advance its defensive capabilities and possibly equip eastern NATO countries with defensive missile systems, right?

My impression is that Russia's antisocial diplomatic posture is a show put on for the large segment of their population who are frighteningly jingoistic, but part of that show is that sometimes Russia actually has to put its money where its mouth is (such as the annexation of Crimea). Whenever the West publicly confronts Russia, the Russian government is basically given a choice between backing down and being humiliated or doubling down and throwing everything at risk for the sake of a strong image. With this in mind, we should be very careful about making disagreements public. I think through back channels we should make it clear to the Russians that if they want a second Cold War we can outspend them even worse than we did last time, but publicly we should continue a policy of engagement on nuclear issues.
