The Russian is a very progressive character and is way ahead of his time
The writer had him come out as transgender, which was a very important statement regarding Russia's backward anti-LGBT laws and opinions at the time and is as relevant today as the day the character was introduced.

Well, either that or the writer just wanted to add two extra boobs to the story.
