Was an explanation ever given for why GG had that nagging cough? I had always assumed it was from Mace crushing his chest in the clone wars microseries, but I dont know if that still holds true.

The explanation now is that the cybernetics implanted in Grievous' lungs irritate the tissue, which gives him that trademark cough:

Same source also states that Grievous' cybernetic transformation/enhancements were all elective. Etc.
