The Groom from the Outlast DLC. Nothing like some good old fashion male genitalia mutilation and a limping protagonist to make you shit bricks.
The Alien from Isolation, as noted, was phucking awesome.

The Site Tau monster in Soma was pretty terrifying. It wasn't even the way he looked. It was the way he moved, how has he was, and the fact that he always knows where you are which means you have to keep moving to evade him. The proxies in Site Theta also made that section the scariest in the entire game for me. (please log in to view the image)

When I first played, the Nemesis in RE3. I'd never played a game with such a persistent enemy before. Even taking in to account the T-90 from Resident Evil 2's B game, this was a different beast entirely, and I used to dread hearing "Staaaars" every time I entered a new room.
The Cyber Demon from, Doom back in the day. Just the sheer dread from having to face off against those bastards with limited ammo make them worth mentioning.
