He was stabbed through the chest with an alien spear in the first act of the film. He never healed or was repaired thoughtout the movie. He was handicapped though 90% of the movie due to injury.
Pretty simple.
He doesn’t have organs and his body is vibranium. Why was it even pierced to begin with. Also he wielded a infinity stone and lost to some aliens cap dealt with pretty easily.
If the spear he was stabbed with was able to pierce vibranium, it shows how powerful the spear was. Plus it was a cheap shot.
His body was injured regardless if he has organs or not. His internals (circuits, connections, etc) were injured. Whatever the stabbing through the chest injured, it clearly affected Vision. Making him not function like he should and not even being able to walk on his own.
Did you not catch that?
Him having an infinity stone doesnt change this. He didnt even know how to fully use it. He admits to this about not even knowing what the thing in his head is.

In the movie it was stated that the weapon he was stabbed with could injure him whether he was phasing or not etc so yeah whatever he was stabbed with was created with the sole purpose of being able to disable/kill him
He got cucked. Wanted to behave more like a human and thus got caught unaware and hit with an alien weapon, which damaged him for the remainder of the film.

Script-wise? The Russos needed a way to turn Vision into a macguffin, as Thanos' quest was a multiple macguffin hunt. In order to incorporate all the characters and settings used in the plot that was constructed, Vision needed a stabbing courtesy of Corvus Glaive.
Also, having another Thor/Superman type in the hero stable might've made the movie too crowded in the big guns department. And Vision got seriously nerfed in Civil War anyways. He was arguably top dog in Age of Ultron, should've soloed both parties in the Civil War airport fight, then gets stabbed in Infinity War.
I like Vision, so maybe he'll come back better than ever in Endgame.
