Consider this. There are some things that are more important than even protecting his secret. Dozens of children are about to die as a result of the actions of Speedball, Namorita, and a few others. Heroes acting on their own, and not having to answer to anyone. And the dead kids? Normally, no heroes would go to jail since they were collateral damage of a suprhuman brawl. So, Spidey has to think: "Is this the type of behavior I'm encouraging?"
He's kept his secret, and his family has STILL been put in danger numberous times before. But if he was to register, not only would his family be under the protection of the Avengers, but SHIELD and the US Government as well. You all talk as if Spider-man is the only hero with family and friends that could die. A number of heroes that hvae gone public have not had their family and friends killed off.

Outside this storyline, the only way Iron Man loses to Spidey is because he's drunk, tired, drugged, and very low on power to the point that anything but movement is going to drain to suit to 0.
