Do you mean pure hand to hand fighting?

Only strength involved, and no other abilities such as Super Speed, flight etc.?

Does Captain Atom get to use his powers to amp?

Does Hulk etc. have ever increasing strength, or is it only at a set level?

Does Hulk still have a healing factor in this fight?

This is probably it, unless Hulk gets to increase his strength, and reaches a level where he can match and eventually exceed Superman. If not, Superman knocks him out, as it his base Hulk starts out at 100 tons only.
