Have you ever noticed Anakin gets his right hand chopped off 3 times!? The first is the duel against Dooku, the second is in a book set after ANH where Luke is looking for some jewel or sumtin', Vader finds him, Luke cuts his hand off, and vader trips over his hand and falls in a hole, and finally is when Luke pwns Vader and cuts his hand off in ROTJ.

The morale of the story? I dunno, you find one.

The second instance you mention is Splinter of the Minds Eye which is EU so doesnt count...
