Strange would be the first to say that Surfer is more powerful than him.

But he's probably wrong.

Given their feats, Strange probably has the upper hand. But sans prep of any sort, Surfer would probably have the advantage in a "Ready? Go!" scenario.

Cold and heartless? erm...where did I do that?

With prep, Dr. Strange has a good chance to beat the Surfer. Without it, Strange is still cosmic, but considering the Surfer's recent upgrade, I'd say the Silver One takes it 9/10.
