Although it's stated to be a collective effort, note that the art in the first scan portrays Arishem alone handling the creation of reality.

Why do people always takes things out of context?

Cassidy couldn't be killed by the bullets because he was already an undead being. It was said that the bullets cannot kill what is already undead.
