He doesn't create a multiverse. He uses you memories to create multiple paths that you, or versions of you, could have taken and uses those paths to warp reality around you, the individual.

As you can see, Kismet's role is to illuminate the many paths between order and chaos for individuals to decide. She originated as a mortal human. Busiek retconned her later to be the embodiment of the DCU, saying that DC had no such character so he had to pick a character to serve that purpose.
