Those had better be some potent grenades to damage the Hulk.
As far as the adamantium swords, she could probably phase them and then resolidify them inside Hulk, but what's to keep him from just ripping them out and getting angrier...
On the other hand, how is Hulk going to hurt a phased Kitty?
Kitty's tried the "fusing the opponent into the ground" tactic once with Thor already. Granted, he was in horrendous pain, but not only did he get out with no fatal injuries, he then proceeded repel an attack from Phoenix.
Hulk's more durable than Thor is and he has that nifty healing factor, so I'd say his odds look better than Thor's do of coming out of the ground.
Thing is though Thor had a magical hammer, where from the looks of it was able to atomically unbond himself from the floor. And looking at that scan again, had she submerged him completely, he would have needed someone else to unbond him from the pavement.
I know this next bit is from a What if, but it kind of shows the effects when she solidifies something into someone. Here her arm is solidified into Wolverines brain. Now imagine an entire person being submerged into a block of concrete.
IIRC Vision is able to phase by altering his density , and if I'm not mistaken regardless of that he still has some sort of total mass in that form. That's how Magneto and Graviton are still able to effect him in his phased form.
Shadowcat passes/manipulates her atoms through the spaces between the atoms/molecules of the medium she's passing through. The denser the material she's passing through the more difficult it is to spin her atoms through the gaps.
Regardless though she doesn't need to make physical contact with the Hulk. All she needs to do is phase the ground he's standing on.
