1. For hand to hand it is very simple - Cain. Cain has pulled off feats reminiscent of Bullseye (when he was in hospital all bandaged and he ripped off a catheter needle with his teeth and tossed it with accuracy with his mouth, and even though wounded managed to absolutely mess up several agents). In hand to hand he was considered sufficiently worthy by Lady Shiva that they had a child together (Cassandra Cain). He's absolutely ruthless, and like Punisher, has tremendous damage soak.
In H2H he is everything the Punisher is, just with superior hand to hand skills.
2. With weapons I'd say the two are equal. The delta of difference between the two is so small that the only correct response is they are equal. They'd probably shoot at each other for a while, and then realize they make better friends than foes and proceed to create a tremendous headache for the police department of whatever city they were dueling in.
