Bol Chatak vs. Siadem Forte

Both jedi went up against RODV Vader. Bol did betetr against him even though Iwo Kulka was helping Forte when he faught Vader, however, Forte and Kulka fought a stronger version of RODV Vader, plus Forte was injured during Order 66, so I'm not sure, though I'm leaning towards Chatak. How was Forte injured again? Was it his leg?
