How did the Executor crash into the Death Star?

Maybe someone here knows, but I don't see how that was possible. The Executor was nowhere near the Death Star right? Yet it crashes right into it?

Is this explained somewhere in a novel or something? Because if you look at the battle, the Executor is so far away from the Death Star and then all of a sudden...it crashes into it after the bridge is taken out.
