All combatants are their strongest versions. All are equipped with their latest toys and gadgets though Strange DOESN't get the time stone. Loki gets his scepter but is not allowed mind control.
Combatants' knowledge of each team is limited to their interactions in the movies, which means they know about as much of each other as they gathered during their interactions with each through previous movies but won't know about each other's newest capabilities as of IW (since they never met during IW). IM and Dr. Strange have already fought together at this point so they'll have some experience of working together.
Fight takes place on Titan. No prep. All are going for the kill. Fight ends when all members of one team are dead.

Dr. Strange with the Time stone and Im easily take this. Thor is a beast but he can't contend with the current version of Strange atm. Strange is really the only reason that IM and crew had a chance on Titan. Loki is the weak link here.

Ah...my bad. Well, still what Strange was able to do against Thanos was pretty damn impressive. Sure, Thor might have the best overall showing against Thanos, but it was ONE attack. Strange was also fighting Thanos (having to deal with the magic/energy attacks) whereas we don't know how SB Thor can deal with that yet. This might as well be a Thor vs Strange fight because the other 2 are largely irrelevant.

Warp out of the mirror dimension? I was under the impression that the MD was another plane superimposed over the normal one and being trapped there was the end unless you yourself are are a sorcerer. If I'm wrong and that somehow becomes useless against Thor, IM and Strange are in serious trouble here. Thor got his best power and durability feat in IW after all, and both are pretty amazing. Otherwise though, Strange gets them the win.
Interesting question. We know the bifrost can travel between realms as well as planets, not sure if that includes dimensions. I.... don't know the answer? I guess it's a valid tactic if it works.
Thor defeated no one. You need to look up what defeat means. Hurting someone doesn’t mean you won. Thanos acccomplished his goal. Thor failed like you do every day of your life I bet.
