WWH was already operating at his angriest(and therefore strongest) levels up to that point -- his ability to mentally snap in times of duress and become stronger still(ie. WBH) doesn't change this.
Point being: the most powerful Hulk we had ever seen at the time was apparently unable to incapacitate Juggernaut, like he incapacitated so many others... and there is really no reason to think that Hulk would have been intentionally holding back the strength he had access to(at the time) against an opponent like Juggy.
...Unless, of course, you're suggesting that Hulk would've needed to enter 'World-Breaker-mode' to definitively overpower Juggernaut? In which case... Okay?

So, you have spent 14 years of your life trying to explain the basics of a Hulk story arc to a black man with a Barbie-like make-up posting cute ducklips selfies on instagram.
Since when after the number of times you quote the opinions of Greg Pak on the Hulk to support your arguments on the Hulk? Are you ready to throw that out of the window now?
Except for the fact that he said said that he was holding back the entire time. You can't ignore context whenever you want to. Bruce held back when he faced Cain. In order for him to have put him down, he would have had to increase his power to levels that would have killed many innocents. This isn't something that is hard to understand.
You realize he had no control over WBH power levels, yes?
And he hasn't even discovered them during that time. Required to suffer a nervous breakdown at the end of the series and transform.
As for his WWH power, he was using most of it, that's how everyone noticed he was superior to SH... he was just holding back not to kill - same as every superhero in the whole damn superhero genre does.
Worrying about crushing Xavier beneath the collapsing building, yeah, I'd call that a handicap.

Cain's history shows us he's always felt conflicted and trying to find his own way.
The hero part of him, X-Men/Excalibur, was still there, despite embracing Cyttorak again.
How do we know that?
He could've very damn well wrecked the mansion and finish the job Hulk started. Instead, he chose to leave and didn't harm a soul - other than beating the living shit out of Banner, of course.
