Cedric Diggory. After Harry brings him back and the band is playing and then his dad starts screaming - that is possibly one of the most tragic moments in all of film history.
Any of the death scenes in Saving Private Ryan were pretty intense. The Medic Wade, and Tom Hanks in particular.
Also...
Boromir in FOTR
Jasper in Children of Men
The end of Pan's Labyrinth
Donnie in The Big Lebowski
Christopher Walken in The Deer Hunter

I can't say it's the saddest one ever, but one that sticks out in my mind is in Harry Potter 4 when Harry comes back with Cedric and he's dead, and his dad starts wailing. I lost it.
hah, i didn't realize someone just said that two before me... so i'm not alone

BELOW ARE SOME MAJOR SPOILERS ON PEARL HARBOR, IT IS NOT MY FAULT IF YOU READ THEM AND GET MAD BECAUSE YOU HAVE BEEN WARNER. PROCEED WITH CAUTION.
[SPOILER - highlight to read]: Alright, what got me in Pearl Harbor, is that Josh Hartnett got Ben Affleck's girlfriend pregnant, when they thought he was dead, and then he comes back, and they fight. So, at the end, Josh Hartnett gets killed and of course dies, then Josh Hartnett's child is born after he dies while Ben Affleck is currently with the girl who Josh Hartnett impregnated, and she (i think) doesn't tell Ben Affleck that it's Josh Hartnett's child. It's an incredible movie and it's really sad. Here's the video they play after Josh Hartnett's death. It's not my type of music but i liked it.

I hated that one. It always made me tear up when I was younger. I haven't seen the movie in about 9 years, but I wouldn't be surprised if it had the same effect on me.
