Help me ID a movie from late 70's

Hello all,

I saw a movie on TV when I was like 7, it scared me to death. I remember two parts very well, but have no idea what it was...

Facts (as I remember them):

1) Black and White
2) People being tortured by monsters (alien looking, big bald head, big black eyes, etc)
3) Guy chained to wall, screaming. Monster is sitting in front of him, sharpening an axe on a grinding wheel. When axe is sharp, he stands up and walks towards the guy....
4) Girl tied to a bed and monster walking towards her with very nasty needle, about to inject her with something....
