Instead of finding Itachi at the hideout Sasuke finds The Kira user Yoite there instead. Yoite came there looking for a way to complete his wish and erase himself from this world, he sees Sasuke and assumes he's standing in the way of his one goal. Sasuke on the other hand assumes this is a trap set by Itachi. The two clash who comes out on top?
Yoite is a FULL health right here, so he's not dying from his Kira,he's not randomly gonna pass out or hack up blood. Neither know of the others abilities. (please log in to view the image)
I'm sure someone is familiar with Yoite but for the ones who aren't here is a quick video of him using his Kira powers there's a bit more to it but you get the jist of it.
