Will there be a viable Palestinian state? How much influence will China exert on the world? How much further will Turkey have gone in joining the E.U.? Will most of Europe have legislated same-sex marriage? Will Africa continue to suffer? Will someone have found an effective treatment or preventative for AIDS? Will Iran be a nuclear power? Will Prince Charles finally be King of England? How much will the planet's climate have changed? Do you think the world will be a safer place or a more dangerous one?
Pessimistic - it keeps you on the edge of things. So you are more focused.

China has a fast growing economy and its armaments and military are expanding. Its to be the next super power.
I see Turkey out of the EU. Its not wanted.
Most of Europe is fast becoming liberal. I hate having same sex couples doing there monkey business publicly. Hopefully NOT.
Africa doesnt only suffer cause western countries are looking down on it. Africa suffers mostly cause of corruption within its ranks. Its imploded. Unless that is first eradicated it will continue to suffer.
There is prevention for AIDS already. Abstension. Yet whoever will find a cure to it may get the Nobel Prize. On the other hand, Its a Man made disease. AIDS keeps world populations from expanding too much. There may as well be a cure for it only that we dont know about it.
I forsee Iran, Europe and USA getting into loggerheads on this.Its already trying to up its Nuclear capacty but the US may very well be undefended at home if it keeps deploying soldiers elsewhere. It will probably call on European States for more military support.
The monarchy no longer controls a country. So might as well put a monkey on the throne. I would worry too much about it.
The Planet will have an abundancy of rainfall.
Ultimatly the world will be more dangerous to low income and some middle class people.

I'm an optimistic realist (they do exist) and it's only logical that things CAN get better, but whether they do? (My money is on even if the obvious problems of today are fixed that the world wont be noticeably "better" or "worse") And then of course there is the reality that usually as things change one gets new problems... still, we can hope... that things get better, not that we'll have new problems

Will the U.S. still be in Iraq? Meh, the US is everywhere, of course with the shifting nature of politics the question is will the US still be able to bestride the world interfering... oops, I mean "helping" places like Iraq?
Will Roe vs Wade be overturned in the U.S. Supreme Court? No idea what this is.
Will India and Pakistan have made peace? Will Kashmir get it's Independence?
Will there be a viable Palestinian state? Hopefully, but then war and hate have become so ingrained in the region...
How much influence will China exert on the world? It's fast on it's way to superpower status in terms of economy, so probably alot, one day it might ever equal the US (if the US is still as powerful at that point)
How much further will Turkey have gone in joining the E.U.? Who knows.
Will most of Europe have legislated same-sex marriage? Hopefully, it's about time nations overcome bigotry and lead by example.
Will Africa continue to suffer? If they can clean up there act? Probably, there is so much work to be done. If not? Yes. And then there is the relation of many Western countries to Africa, if they to stop exploiting the region...

How much will the planet's climate have changed? We've already got an increase of tropical storms and droughts, I dread to think...

I don't know if the world really gets worse. I mean if, in the stretch of history, you compare modern times to say the Roman Imperial era, or during the Industrial revolution, or even from 1910-1950, it looks as if things have improved, or at least haven’t gotten vastly worse. Depending where you are life spans have increased, child mortality is reduced, your not quite as likely to starve or die of disease, you can get an education and hopefully a job, law is better defined and defended, you probably won’t be slaughtered by some marauding army or sold into slavery or eaten by ravenous beasts, there’s global communication, medicine works, multiculturalism continue to go strong, the concept of human rights exists... and so on, unfortunately that is not the whole world, there are still huge swathes where things are very bad, though anthropologists would argue that even there such nations have benefited in some ways from the progress of time, and with more time even they could be good.
Of course with overpopulation when bad things happen it is terrible, and looks even more so, though the modern world is much more capable of dealing with such disasters then it was in the past (even to the extent such things could be predicted and planned for)
