He got his asswhooped during Final Crisis by 2 Supermen that was kinda meh though.....and Rex was also KO'd as for Juggs as far as I remember only Onslaught has ko'd him but that time he was kinda weakened IRRC.....now Hulk has been ko'd countless times.....
That whole fight was...dumb. And the Onslaught thing was also weird. Because Cain had tossed the Cyttorak gem into space years before Onslaught even formed. And you're correct, he'd been weakened a great deal after losing a lot of his powers to escape that realm he'd been trapped in.
He's been KO'd one other time, but that was Captain Universe using Captain Universe level telepathy as a sneak attack.
Onslaught koed him/Juggernaut and the sad thing about it is, Hulk fought a more powerful version of Onslaught and not only physically dominated him but punched his shield to dust. Prime one shot koed Doomsday as well.
The winner's either Juggs or DD, since Hulk would quickly tire out moving against such a massive gravity. Of those two, I'd say in the end the winner would be...
