A while ago, she supposedly crashed in a plane accident and Peter was devastated. But then he found out she's alive.

In the 90s cartoon, she fell into another "dimension" during the struggle between Green Goblin and Spidey. Then a MJ clone arrived, who married Peter, but then later turned out to be made from Hydroman's genes. She evaporated and that was the end of her. The last episode has Madame Web telling Spidey they're going to find the "real MJ."

MJ doesn't die. Peter and she are married, and Peter would never let anything happen to her... unless in an alternate reality.
