No jjwil76, the image is not a modified Hayden photo.
I found the head image on the website of the company
that did creature makeup effects for Revenge of the Sith.
I added extra burns and scars,then added layers of
a air mask,then about 8 layers of transparent bubbles.
The head image is actually of work the company did
for the series Farscape, so I thought that maybe they
might do similar work with Anakin. Here is the site,

damn, they do some creepy work. i tried to do that one halloween when i went as count chocula and it was a nightmare. prosthetics fell off 10 minutes into the party. thats a nice site for monster pics that can be altered (for private work, of course).
