The only 100% reliable method is to have no sexual contact with anyone whatsoever..... then pregnancy and disease just don't happen... otherwise, I'm afraid, there's always a chance....

Yes, other wise, if both parties are certain they are not carrying an disease, or if she is prepared to take the risk of becoming pregnant, then a condom isn't necessary, other wise yes. While it is true they may not stop things 100% of the times, they are the safest, and one of the most effective form of protection, things such as withdrawal are not...
