I wouldn't think there's only four. Hades is from the Greek pantheon, Hela from the Asgardian, what about the others like Azteks, Celtics, Egyptian... each would have their hell-ruler equivalent. And I think Hades and Hela oversee the dead and death-realm of Greeks and Asgard, respectively - I don't think it's "hell" per se.

This is correct. Hela and Hades mostly deal with the "unhonored" shades who didn't have the distinction of being great warriors or noble folk. However, they do have (at least from the myths) souls that are being punished for crimes against the gods (e.g., Tantalus and Sisyphus of Greek Legend)
As for a supreme ruler of hell, most of the demons are a lot weaker outside of their realms(Mephisto especially). It would be pretty hard for Mephisto to establish dominion over another demon (Satannish or Hellstrom) through pure force.
These guys would have to come up with a major plan (like Mephisto in his series; he tried to unleash the soul of THor into Hela's realm to weaken her and get a larger share of souls). This plan was so elaborate it took 4 books in a series to flesh it out.
From what I have heard, though, Lucifer of DC is (correct me if I am wrong) beyond the level of Galactus. If this is true, then he would be the definite candidate.
