PAC man is actually a space soldier running around a huge space ship being chased by imaginary ghosts and the little dots are actually anxiety pills so he will stop seeing these imaginary ghosts. The fruit are actually his friends that are alive but since PAC man is eating so many pills he kills them. When you get all the pills the ghosts disappears and the screen slowly blinks indicating that he is slipping away into a coma

That's a pretty disturbing prospect
