Because it's obviously something that still affects him personally. Regardless if people know he's bald, he doesn't want to be so now has the chance to change that.

He's just having cosmetic surgery. He even said himself that at his stage of baldness, the transplant is not going to give him a lot of hair, he'd most likely still have to one of those old guy comb-overs.
It's like getting a boob-job at 52. It's just not relevant anymore. awehuhs

If it bothers him then it will always bother him and like Welshy said maybe only now he can afford to change it. If he feels women won't be attracted to him because of it then he will want to change it. Regardless of how long he's been bald.
It's a personal thing. I'm totally for people changing things if it helps them feel more confident.
