I can't recall any instance in which Hulk has been hurt by regular bullets.
Before the healing factor introduction, Hulk was treated as a nigh invulnerable being for the most part similar to Superman/Surfer so even back in the old school days he was bullet proof. It still goes back to that approach sometimes for a very long time Wolverine's claws couldn't bypass his invulnerability. Even in the late 90's, IIRC it was only due to Hulk's own momentum that Deathverine was able to cut him.
And I'm not sure whether that's Hulk pre Savage Banner or not. That incarnation would wreck any character short of Superman/Thor physically and he wouldn't even have to get very angry, Pad wrote him as generally above Savage Hulk physically, basically an old school Green Scar.
For the record, the incarnation that Superman just barely beat was after the Savage Banner limitation was introduced.

Yup, why wouldn't they be? It's true that they get a little silly (That herald business for example) but it's all in continuity last time I checked.
It wasn't a Star as I recall.
