You know, the Rule of Two makes no sense. His entire Rule of Two idea... depends on the Sith acting against their interests.
I'm a Sith Lord, I defeated my master and took his place. I'm a Sith; I only care about personal power and enrichment. Why would I take an apprentice? Why would I share my power and knowledge, why would I train up someone to be a threat to me?
The entire concept of the Rule of Two, depends on sacrificing personal gain for someone else's distant revenge. All you would need, is for one Sith in the chain from Bane to Palpatine, to decide he doesn't give a shit and to not take an apprentice.
Boom, game over. You lose, you do not pass go, you do not collect two hundred dollars and kill all the Jedi.

More or less. Requires a sort of forward-thinking that Sith aren't known for. In a way, the Sith are boned from the start: the same dark side-induced psychotic lust for power that precludes them from working well en masse is the same sort of behavior that would make the RoT very difficult to implement.
The Sith are more or less a bunch of really dangerous losers. Palpatine's their one and only MVP by a mile. Not exactly the kind of record I would personally want to attach myself to.
