Banner’s restrictions were mostly keeping Savage Hulk from becoming insane with rage like Mindless. Here, however, no Banner caused Hulk’s body to break down.

Are you asking if he ever died the whole time he was separated by Banner or just during that fight? ‘Cause he did almost die just before Maestro made his return. His body just broke down and he was fine again when Banner returned.

It’s hard to say whether he ever has actually hurt Surfer or not, even in that fight. We’ve seen Hulk beat Thor down in the ground just as badly as he beat Surfer’s head in there. Thor got back just fine but I wouldn’t say he was unhurt. We’ve also seen him not do any noticeable damage to Thing before, even in fights where he dominated.

He was stabbed by an ordinary knife around this time, but then again this may have only affected his durability. He did have a lot trouble with Mr. Hyde, someone who can’t even register with Thor any more. He even showed signs of weakening in a fight with Wolverine just before a T-Rex threw him around.

Surfer usually shows some form of pain when he gets hurt. He isn't fueled on pride. In that fight, he didn't show any, and he still wasn't trying.
I find it hard to believe that he got hurt.
