I get that there'd be a backlash, magic or telepathy gone awry tends to do that - and that death on the astral plane means the body basically dies mindless. What I don't get is the direct translation of a wound, inflicted upon what's essentially a mental representation of Strange - if he wanted to he could have appeared as a talking octopus in a cape to Hulk, as a physical wound on Strange's body - it would have made more sense had his hands been crippled mentally rather than physically.
Because Hulk has a history of doing feats like this. Call it PIS or not, but he can punch his way out of a timestorm, thunderclap and destroy a pocket dimension, and use his hands to peel away magical energy.
We even see when he turns into Warhulk that he uses a nimbus of green energy when going against the Juggernaut. That shows that latently there must be some other power at work other than purely physical.
If he was a purely physical entity none of those feats would be possible.
