Thanos didn't outsmart the runner, he already had the time gem. It's not hard to beat someone when you control their age.
Some of that was PIS, especially the one with the Runner. The Runner is immortal he should not age. Hell didnt he do the same **** with the Gardener that shouldnt have happened either.

It would be like someone drank from the fountain of youth, and then all of the sudden, someone came along and aged them up by a hundred years.
Also, the context of the story, makes it seem like Thanos aged his body like that. Considering he did nothing to time, other than the effects on Runner's body.
