I doubt that would work, Kitty's(Spin her atoms) phasing is different to Visions phasing(Density Control). And she was successful in WWH: X-Men.
Besides she doesn't need to make contact with him to phase him. She only needs to phase the ground under him.
Bust right out? How would he if he wouldn't be able to move. His molecules, his organs would literally be fused to the ground. His entire body submerged underground. He's body would be splinched between the concrete atoms. When Kitty phased Thors bottom half into the ground, he had to unfuse himself using Mjolnir.
He's nervous system would be splinched as well...meaning that he wouldn't be able to move, he most probably wouldn't even be conscious with his brain being splinched.
Thor and Hulk work differently, it's been shown time and again that Hulk's body reasserts its shape/form regardless of transmutation/matter manipulation.
the specifics don't in anyway encumber this, because we do not know enough about the nature of this ability of Hulk to suggest that it can be counter acted by one specific method.
what we've seen with respect to Kitty was that, when she phased his fists into the ground - while he was in a calm state - he just 'broke' out... hands intact.
what's that mean? that his body will always reassert its shape/form, imo.
check what Zom/Strange did... a massive friggin' hole right through his abdomen, no 'organs' to repair - a hole!
what happened next panel, he healed up, reconstituted whatever was missing and then had it busted open again by Zom/Strange only to heal right up again (this time while buried under a collapsed building).
I think the crucial factor in all this is that Hulk gets his matter from another dimension/universe... such that +everything+ can be replaced... or more matter can be 'injected' into a space (in 'this universe'😉 thus pushing out whatever's there...

It didn't seem to be any different in World of War Hulk: X-Men #2. Hulk had pieces of Concrete fused into his arms and legs once he broke out. Had she submerged his entire body into the ground. For all purposes the Hulk would be dead. 😐

He was in extreme pain when she phased him...just like Thor. Again since his entire body wasn't phased into the ground he was able to break out...with pieces of concrete sticking out of his body.

Not necessarily because his nervous system wasn't completely damaged when she did that. Had she submerged him into the ground. There would be no nervous system and no biological activity. Therefore no movement. No brain...no movement, no healing factor..etc..

He still had his cells and brain though. His cells contain his Healing factor...and his brain regulates his healing.

Even with his brain dead, he goes on. He is probably one of the top regenerators of M.U. right now in his WWH form.
It has been shown in the past he was able to resist: Being turned to stone, being shrunk down (he grew right back up to normal size), and any other type of transmutation.
When you're made of glass, you have no nerve cells, correct? Well guess what, he survived and resisted being trasnmuted into glass as well. Even with his healing factor supposedly taken away, it came right back in a few pages. His body automatically re-asserts itself no matter what. The exact mechanism is unknown, so you can't say for certain that it's his nerves cells or what-not that is doing it.

Since he's resisted the attacks of one-trick ponies, he's automatically immune to transmutation? Interesting. Especially considering how Thor, Spidey and several others have been able to overcome GG's petrification easily.

Maestro Hulk dies, in the original gamma explosion (travelling back into time) but then he does reform.
Hulk's molecules, atoms, whatever, have the same basic properties we see with the Hulk's overall body. incredible strength, ressilience and healing...

IRRC Mindless Hulk is Hulk without Banners consciousness. Mindless Hulk is still able to perform biological movements such as walk, see etc...therefore he does have to think on that level.

It wasn't an electrical storm though...and nope it wasn't PIS....as I said it was a combined effort by Storm and Cable. Storm attacked his nervous system with lightning. Then Cable used a mind blast to further short circuit his nervous system and knock him unconcious.

Yeah but none of those involved being attacked by a mind blast. Which the Hulk that appeared in the Onslaught Saga wasn't immune to.
Again it shows that Hulk relies on his nervous system to carry out normal everyday biological functions.
