There's very few things that would lead to an immediate, no warning permanent ban. The most common is socking, or someone who spams immediately upon joining.

Then how come every permed account I went to, there was only 3 bans in total.

Somehow I doubt you've seen the profile of every single banned person on KMC.

Yes, usually a permban is earned after or about the third ban. However, it's not always the case. It's a case by case thing and is up to the discretion of the mods involved.

Awesomeness factor is surely considered as well.
