Surfer would beat her more often than not. But it will take much unless he transmutes her.
He is faster than her, although SS hasn't shown great combat speed and it's possible KOS-MOS can match his combat speed.
He is a class 100 and can amp his strength via Power Cosmic.
Durability goes to him. He can survive Super Novas and the like.

On the other hand with the Tertiary Weapon system she can protect herself from Surfer's blasts. Her shield was strong enough to save the party from U-do's blast.
If she hits him with the blast she destroyed Galaxies with the Surfer is a goner.
And she still has the phase transfer canon that can vaporize solar systems.

With that said I say Surfer wins, but he better not be his pacifist self or he will get blasted to oblivion and beyond.

Problem with KOS-MOS is that there are too many versions of her.
Ver. 4 definitely has durability up there.
