Are you saving or harvesting the Little Sisters?
Good= you save all little sisters + bonuses (that's where it shows Jack adopting them)
Bad= Harvest at least one little sister (submarine is attacked by splicers)
Angered= Harvest all little sisters (same as bad but the Dr. speaks in an angry tone)
Fountaine pumps himself full of ADAM and you use the little sisters to stick those needles into him, and you get to pilot a Big daddy, it really is like System Shock.
Oh and to answer your Q I am going for good ending by saving every little sister.
