I think he only became older because he absorbed all that energy from that Guardian of the Universe who sacrificed his own life at the end of The Sinestro Corps War saga to destroy Prime (which actually resulted in Prime being warped into the Multiverse atom by atom) and because of that he aged rapidly as his muscles became swollen with power.
However, as Mr. Mxyzptlk told him...Prime's newer, older form was only temporary. The power he took from The Guardian was depleting and by the time Final Crisis: Legion of Three Worlds happens we can pretty much tell that all of that extra power is gone since he looks like a fifteen year-old boy again.
Nevertheless...he still likes to call himself Superman-Prime no matter how old he looks. Hopefully that answers your question. Oh and before I forget...this thread rocks. Kudos to the Threadmaker!
