It's really never like that. Every team has one or more firms. Hardcore supporters. For example Lazio Roma have the Irriducibilli, a pretty famous group of hooligans with fascist ideas. They really would die for their club. They plan to fight another firm before every match. When they play their biggest rivals AS Roma they go all out.

Although obviously it is idiotic, football violence never 'just happens' it's kind of an organised crime.

Yeah, but especially in Italy and England, a club is part of a city, neighbourhood, part of it's identity. The clubs usually originate from around 1900, and from then on people will support it, their children will support them and their children will support it etc... the bond grows stronger. To them it is what saying ''I would die for America'' is to an American soldier.
