Okay, so I was thinking about this all night a few nights back, and I really wanted to make these things, so here's two I've done, and the templates for all four types. I need to know template number and all the information asked for on that particular card, as well as needing a pic for it.

Umbrella Corp. Simple (Template #3)

Umbrella Corp. Detailed (Template #4)
