3 times out of the 4 were done by amped characters.
Without any amps, is there anyone in either/both Marvel & DC besides MM capable of breaking it?
=Entities/Gods don't count.
Anyone with matter manipulation or high-level magic.
For instance, in DC Firestorm could easily turn Captain America's shield into potato chips, and in JLA: Obsidian Age the monster who had absorbed Zatanna used her powers of speaking backwards to turn promethium (the DC equivalent of Adamantium, which Firestorm had created out of thin air to bind her) into PAPER ....simply by saying promethium to paper backwards.
There are many more who can do similar in DC, and just as many in Marvel.
The only place where the shield holds its own is against direct physical strength, and normal energy attacks.
Although even in pure strength there are some characters who can seriously affect it, even to the point of damaging the binds.
For instance, Emo-prime at his best could wreck the shield based on what he was shown doing.
Remember ....it is just that the molecular binds in that shield are so EXCEEDINGLY strong that nearly nothing can even dent it, but what if a character has greater strength than the valence of the molecular bonds in the shield?
The shield breaks!
There are certain brutes (like Emoprime) who could very well just break the shield using strength.
And there is a list of matter-manips and high-level magic users who could simply turn the shield into goo.
It is just that Captain America seldom faces off against such ultra-ubers, otherwise he would have lost his shield quite some time back. ....
